There are risks associated with visiting rural properties (where we are located), and visiting anywhere at all during a pandemic. Potential risks include (but are not limited to): encounters with dangerous flora and/or fauna (e.g. snakes, ticks, coyotes, dogs, deer, falling tree branches, poison ivy, etc.), drowning, falling off swing sets, potential exposure to illness, hunting or farming accidents (if you wander off the property), etc. Submission of this form indicates that you accept all risks involved in coming to the Monaco property and you agree to indemnify and hold harmless the Monaco Family and Music Studio for, from, and against any and all liabilities.
An invoice for the first month's tuition will be sent upon receipt of this form.
